Note: To terminate the NETVIGATOR services (including extra services, broadband or 56K Dial-up service), please submit your termination request at least 30 days prior to your requested service-end date. If you terminate the services within the commitment period, you will have to pay for the early termination charge and other charges according to your service contract.

Please note that in the event you do not provide the Data marked with asterisks we may not be able to process with the matters under this Form.
